Chapter Four:
HTA in a World That No Longer Waits
Health Technology Assessment was built for patience.
Not slowness.
Let us speed it up: deliberation.
Time to observe.
Time to compare.
Time to decide.
HTA emerged in a world where evidence arrived late
Outcomes
unfolded slowly, and uncertainty
was an accepted cost
Its power came from distance,
standing back from the noise
But now, that distance
is disappearing
There is no longer a stable decision point!
Evidence flows, it doesn't arrive
Outcomes accumulate, confidence moves!
(oh, it moves) Scarcity is over,
the famine years are gone
Tick tock, tick tock.
Not the social media.
No.
Curative medicines
ask HTA to watch
Watch patients improve,
watch trajectories resolve
Watch uncertainty collapse
year by year
One requires imagination!
(imagination) The other requires accounting And accounting always wins!
By the time HTA finishes debating assumptions,
The system has already observed reality.
This is not a failure of diligence.
It is a mismatch of tempo.
HTA was built for a clock that no longer governs healthcare.
